A free Secret Santa questionnaire to print or save as a PDF: 20 questions for adults and a kids’ version with tick boxes and space to draw. You don’t need to sign up or give an email address.
Each person fills in a sheet before the draw. Whoever draws their name then knows their sizes, favourite things, allergies and what to avoid, without asking and giving the secret away.

How to use it in a family exchange
- Agree the budget per gift and the exchange date first, and write them at the top of each adult sheet.
- Print one sheet per person, or share the PDF in the family chat so everyone can print their own. Children fill in the kids’ version with the adult who looks after them.
- Hand the sheets out before the draw, with the invitation. Every question is optional.
- Keep the filled-in sheets where every giver can read them: in a folder at a family gathering, or as a photo of each sheet in the family chat. A sheet says nothing about who drew whom, so sharing all of them keeps the draw secret.
- Leave addresses, school names and phone numbers off the sheets, and arrange deliveries between adults.

- Yes. Print it or save it as a PDF as often as you like. You don’t need to sign up or give an email address.
- How do I save the questionnaire as a PDF?
- Select one of the print buttons, then choose Save as PDF as the printer or destination. Some browsers call it Save to PDF. The adult sheet and the kids’ sheet each start on a new page.
- What should a Secret Santa questionnaire ask?
- Sizes, favourite colours, snacks and hobbies, things the person already has too many of, allergies, a few ideas within the budget and the gifts they would rather not get. Every question is optional.
- Can children fill it in?
- Yes. The kids’ version uses tick boxes and a space to draw, and a grown-up adds sizes and allergies. Keep school names and addresses off the sheet.
- When should we hand it out?
- With the invitation, before the draw. Givers can then read the answers as soon as they know who they are buying for.
Keep the answers in the app
